Online scheduling is available for selected appointment types. If you need a different type of visit, are unsure what to choose, or prefer assistance, please call or text our team.
Please Add Your Insurance Information in the Comments Box
The scheduler does not have a separate insurance section. On the Patient Information screen, enter the following in the “Comments or special request” box:
- Insurance company name
- Member ID
- Group number, if available
- Subscriber’s full name and date of birth if the policy is under someone else
Providing this information helps our team review your benefits before your appointment. Please do not enter a Social Security number or payment-card information in the comments box.
Benefits, eligibility, copays, deductibles, and patient responsibility vary by plan. Benefit information is an estimate and is not a guarantee of payment by the insurance company.
How Online Scheduling Works
Select Your Visit
Choose whether you are a new or existing patient and select the appropriate appointment type.
Choose a Time
Select one of the available dates and appointment times shown in the scheduler.
Enter Your Details
Complete the patient information and place your insurance details in the comments box, if applicable.
Book Your Appointment
Submit the form to add your selected appointment directly to our office schedule.
What Happens After You Book?
Your selected appointment is added immediately to our schedule. Please expect a confirmation text or call from Country Club Dental before your appointment so our team can confirm the details and answer any questions.
